Acetyl hexapeptide-38 is a synthetic chain of six amino acids, and it targets something no other peptide in common use does: the fat layer beneath the skin.

Why fat matters to facial ageing

Discussion of skin ageing concentrates on collagen and elastin, and reasonably so. But subcutaneous fat is a substantial contributor to facial shape, and it does not simply thin uniformly.

The face contains discrete fat compartments that change independently with age. Some lose volume, others descend or redistribute. The result reads as hollowing around the temples and midface, flattening of the cheek, and shadowing that was not there before.

This is the change that dermal fillers address directly, and it is largely untouched by ingredients aimed at the dermis.

The proposed mechanism

Acetyl hexapeptide-38 is studied for effects on PGC-1α, a transcriptional coactivator involved in regulating cellular energy metabolism and, relevantly here, in adipocyte differentiation and lipid storage.

The proposal is that supporting this pathway encourages adipocytes to increase in volume, restoring some fullness to areas that have lost it.

That is an unusual direction for a cosmetic ingredient. Most ingredients acting on fat cells in cosmetics, caffeine among them, aim at the opposite: reducing lipid content in body contouring products. This one aims to increase it, in a specific facial context.

The evidence, stated carefully

Research comes from the ingredient manufacturer, which is the norm for proprietary cosmetic peptides, and independent replication is limited.

There is also a more fundamental question than usual. Adipocytes sit in the subcutaneous layer, beneath both the epidermis and the dermis. That is considerably deeper than the fibroblasts most peptides target, which are themselves a delivery challenge.

Whether a topically applied peptide reaches the subcutaneous fat in meaningful quantity is a genuine open question, and it should be weighed against the fact that the alternative approach to the same problem, injectable filler, places material exactly where it is wanted.

The realistic framing

This is a supporting ingredient in a formula, not a substitute for volume restoration procedures. Products containing it typically describe supporting the appearance of fullness rather than restoring volume, and that distinction is doing real work.

Why the acetyl group

Acetylation makes the peptide slightly more lipophilic and improves its stability. It is the same approach behind acetyl hexapeptide-8 and acetyl tetrapeptide-2, and it partly addresses the barrier's exclusion of water-soluble molecules.

In formulation

Used at very low concentrations, water-soluble, stable, and typically combined with other peptides aimed at different targets rather than used alone.
